Consent to the Processing of Personal Data

I hereby grant my consent to the N.N. Petrov National Medical Research Center of Oncology, located at the following address: 68 Leningradskaya Street, Pesochny, Saint Petersburg, 197758, and its authorized representatives to process my personal data provided by me in the patient’s registration form as well as any other data relating to my person (hereinafter referred to as the “Personal Data”); to perform any action (operation) or a series of actions (operations) towards my Personal Data, including (but not limited to) their collection, recoding, systematization, accumulation, storage, adjustment (updating, alteration), retrieval, use, transmission (dissemination, sharing, provision of access), anonymization, blocking, deletion and destruction; to perform any other actions stipulated in the current legislation of the Russian Federation, with the use of means of automation, including information and telecommunication networks, or without the use of such means of automation, provided that the processing of the Personal Data without the use of such automation means harmonizes with the nature of actions (operations) performed towards the Personal Data using such means of automation, i.e. it allows, following a pre-determined algorithm, to conduct a search of the Personal Data which are fixed in any tangible medium and stored in an archive or any other organized personal data collection, and/or to access such Personal Data; and to transmit such Personal Data (including cross-border transmission) to authorized representatives of the N.N. Petrov National Medical Research Center of Oncology and to third parties who are its contract partners, including medical and research organizations, communications service providers, Internet service providers and consultants.

I hereby confirm that the Personal Data which I have provided to the N.N. Petrov National Medical Research Center of Oncology are adequate and can be processed by the Center and its authorized representatives for medical, research and promotional purposes.

I hereby give my consent to the N.N. Petrov National Medical Research Center of Oncology to send e-mails/messages to the e-mail address and/or mobile phone number provided by me, which may contain information on organized events and other information, as well as send targeted promotional materials to the e-mail address which I have provided

I agree that the text of the consent I have granted voluntarily and in my own interests is stored in electronic form on a database and/or in paper form, and demonstrates the act of my consenting to the processing and transmission of my Personal Data in accordance with the above, and I shall bear the responsibility for ensuring the adequacy of the Personal Data provided by me.

This consent is given for an undetermined period of time and can be recalled at any time by sending a written notification to the N.N. Petrov National Medical Research Center of Oncology, at the following address: 68 Leningradskaya Street, Pesochny, Saint Petersburg, 197758, Russia.
